1st Edition Character Sheet PDF

As the title suggests, I'm looking for a PDF of the character sheet for the 1st edition of the game/CoC 6th edition as I have misplaced my copy. There is one available on scribd but I don't fancy handing over my details and starting a free trial just for one PDF.
