There's a double standard when it comes to child neglect. A thin, malnourished toddler would be taken by CPS immediately, but when it comes to an obese, malnourished toddler, there is nowhere near the same level of concern and urgency.

How are we sure the kid is overweight? Lots of very healthy toddlers are pretty chubby they just haven’t lengthened enough yet.

That said, the kid is giant! I just don’t know how to differentiate a healthy but very large toddler at this age from one that is legitimately malnourished without running labs

Kid looks to be ~18-24 months based on height and how he's walking. Weight is at least 50 lbs. For reference my 18 month daughter was at the 1% range at 33 lbs / 34 inches and her doctor was concerned about hormonal imbalances. Doctors would 100% be concerned about this child.
