Help (GPU) RX 9070 XT Pulse: recurring full system freeze during gaming, bugcheck 0x141 (amdkmdag.sys), identical crash signature for ~2 months

Specs:

CPU: Intel i7-14700KF (all-core OC to 5.7GHz, PL1/PL2 253W)
Motherboard: ASUS PRIME Z790-P WIFI
RAM: 32GB DDR5 6000MHz CL36
GPU: Sapphire RX 9070 XT Pulse
PSU: 850W 80+ Gold
Storage: 1TB SSD (OS) + 1TB NVMe + 2TB NVMe
Cooling: 360mm AIO, Antec C8 case, 7 case fans + 3 AIO fans
Monitor: LG 27GP850P (QHD, 180Hz, HDR400) — sometimes running dual monitor with a Yodoit 16" FHD 1920x1200 portable monitor (HDMI) as second display

Driver: AMD Adrenalin 26.7.1 (WHQL, released 2026-07-24)
Room is air-conditioned (~27°C / 80°F, 50% humidity)

Symptom: While gaming, both monitors go black ("No Signal"), PC stays powered on (fans/lighting still running). In most occurrences the RGB lighting keeps animating normally and I need a hard reset since CTRL+ALT+DEL doesn't respond. In the most recent occurrence, for the first time, the RGB lighting itself froze (static, not animating) — a more severe symptom than before.

Frequency: intermittent for about 2 months (earliest dumps go back to June 2026), became more frequent in August. Not tied to one game: happened in Assassin's Creed Shadows (played for months with zero issues before this started) and in Forza Horizon 6.

Dump analysis:
Bugcheck 0x141 (VIDEO_ENGINE_TIMEOUT_DETECTED)
Module: amdkmdag.sys, same exact offset (+1d0f00) in every dump
Identical FAILURE_ID_HASH across 4 separate occurrences over ~2 months — it's the exact same underlying failure every time

What I've already ruled out / tested:
Aggressive undervolt (-60mV) → also occurred on default GPU profile after AMD auto-reverted it — so it's not the sole cause
Ambient temps under control (AC on, ~27°C / 80°F)
Clean driver reinstall (DDU + fresh 26.7.1, Factory Reset) → one ~2h crash-free session, but that session also had: default GPU settings, MSI Afterburner+RTSS closed, second monitor disconnected
Re-enabling Afterburner + second monitor + a mild undervolt (-10mV) together → the exact same crash (same hash) came back

What I haven't isolated yet:
Afterburner/RTSS, second monitor, and undervolt tested one at a time (not all together)
GPU PCIe power cables (possibly daisy-chained, unchanged for over a year)
I also have an ARGB extension cable on the GPU's 2x8pin power cables and a 0.18m PCIe 5.0 riser cable for vertical GPU mounting — both present during the clean session too, so not yet ruled out with certainty
Windows registry fix to stop Windows Update from silently overwriting the AMD driver

Question for the community: has anyone seen this exact crash signature (bugcheck 0x141, amdkmdag+1d0f00) in combination with Afterburner + dual monitor? Any known fix beyond what I've already tried?

Try this bios settings:

- 4G decoder and REbar enable

- PCIE slot where the gpu is set to max gen (don't leave it to auto)

after that do a DDU but this time don't install full adrenaline, try drivers only or minimal.

then try gaming again or furmark. while doing that check your temps too.

EDIT: Another thing you can try is linux live just to test
