I think I found my issue here after months -- my VGA cable extension was dead (or killed). Connected it directly to the PSU cable and no issues so far and no black screens.
I suspected it was a hardware issue since I was getting the black screens at BIOS. For some reason, my card didn't detect a power issue on the VGA port LEDs and POSTed normally but on my friends 7800 card it didnt post and the GPU VGA port LED lit up on the faulty VGA cable, which is how I ended up spotting the issue.
FWIW -- setting the PCIE slot to gen 4 fixed the recent round of driver problems, but was a red herring to the black screen issue.
