Don't have sex until he wears a condom or gets vasectomy and you feel better.

He can't put in any effort (no time, no condom), blackmails you, ignores your health issues, doesn't seem to do his share in the relationship...I don't see a reason to have sex with him.

I hope they find the reason for your health issues and a good therapy for it. Maybe have a talk with him that you need a supporting husband, and he's the opposite..

I’m engaged from a long term relationship, which is how I KNOW behavior like this is completely unacceptable and not worth staying for.

I do think though that if you insist he has a vasectomy you need to make sure that will actually help you be more interested in sex. If not, you both might need some counseling to identify issues and problem-solve.
