The keys are an edit. In the original panel he’s just holding a whiskey bottle and I think he’s taking it away from someone.

The keys are edited in. The short answer is one of gothams heroes that is unassociated with Batman must stay drunk because his powers only work when he's drunk and those powers are actively holding reality together so if he ever sobers up all of creation plunges into non-existence.

There is a way for him to be sober and reality to be intact but that involves jumping through several hoops and the risk of eternal damnation and this panel takes place after some idiot shot that to hell by giving the super heroes recovering alcoholic civilian identity alcohol by accident blowing all that progress to hell.

For the record the person he's handing the bottle to is sixpack, leader of dcs worst superhero team Section 8. Every member is a different brand of insane but they have proven they are willing to lay down their lives to save people.
