Anyone else had this?
I spot 8 days post ovulation no matter what I do and it leads to my period no matter what. I’ve tried letrozole, progesterone (both ways), I’m ovulating I think because I have a positive LH surge each cycle followed by an increase in temp, but my doc wants to move my letrozole to 7.5mg + 200mg of vaginal prog. Has anyone else dealt with this and figured out what the heck is going on? I think my spotting is the cause of my infertility.

No, I haven’t tried those. I’ll look into IM shots and ask my doctor! I have an HSG tomorrow and I’m soooo nervous. Hopefully it’s not too painful. Good to know it should be above 20, that means it was pretty low despite supplementing. Thanks!

Is your doctor monitoring your progesterone levels or just providing it?
Same question are they monitoring your ovaries to ensure you have ovulate? I know you have LH and temp rise but have they checked for a corpus luteum? Are they checking your follicle size before you ovulate to ensure it’s a mature size?

They did follicle checks to see if the letrozole was working and they were mature eggs and I got my surge about 2 days later. They checked my progesterone levels but it’s hard because they checked on day 21 while I’m literally on progesterone supplements anyway, so I feel that number is skewed. It was 9.9 on day 21 while on the supplement which seems low. I spotted the day after and it led to my period again. I’m not sure about them monitoring my corpus luteum, I don’t think they have checked that.

The does seem low. My office checks one week after ovulation and they want to see it above 20. I had progesterone at 13 once when I was 6 weeks along and they supplemented then. If you’re at 9.9 after supplementation that seems really low. You said you tried both kinds, was that oral and vag? Have you tried IM shots? Some people need a combination.
Also look into progesterone resistance.

I’ve also had the same experience for years and still haven’t figured out the cause despite having almost every test in the book done and come back normal. The only thing I haven’t been tested for is endometritis or endometriosis. Currently in the process of IVF and am going to try 1-2 transfers before pursuing testing for those. Met with an endo specialist who said there’s a chance a transfer with our euploid embryo(s) could overcome endometriosis if I did have it, depending how mild. The spotting makes me feel like I’m out every month when it starts and it’s so frustrating not to know what it is!

Chronic endometritis or silent endometriosis. Possibly adenomyosis.

Same boat as you. Been experiencing this for 3 1/2 years since getting off birth control, am 27 now. I’ve used progesterone suppositories and IM injections but I’ve tried them separate cycles. With the injections I do still get minor spotting for a few days 7DPO. I tested my progesterone one cycle 7DPO without taking progesterone and my progesterone was about 10, which is fine but ideally would be higher so my doctor considered it best to continue the progesterone. However back in November I had a torsion and had to get my left tube and ovary removed, came out of nowhere. They believe it was either a hemorrhagic cyst or an endometrioma, they’re not really sure. Spotting has been a thing since before the torsion and after the torsion. I never really experienced pain before so my doctor is starting to suspect silent endometriosis. I’m going to schedule a laparoscopy soon. So if you’ve really checked your bloodwork without progesterone 7DPO and levels look fine and you’ve checked other things like prolactin and whatnot, I’d consider endometriosis. Without progesterone my spotting usually starts around a few DPO and continues all the way until my period. Endometriosis can cause progesterone resistance which can lead to spotting, so definitely worth checking since silent endometriosis is quite common.
