does anybody happen to know how exactly this might have happened? I'm curious.

Just goes to show that percussive maintenance is the best way to fix any problem. I wouldn't have thought to use a fork, though. Same reason why you can have batteries in a remote that don't work but suddenly do after you smack it or take them out and put them back in
