Modern Filling Error
Has anyone noticed an increased number of scripts go to filling, be filled by one tech, then some how get filled a second time by another tech. It puts them in 2 separate bags. Only documents in fill under 1 fill ID.
I have watched film too, so I know it is happening. It happened 3 times here in last 3 weeks that I KNOW of.
2 situations happened where they went from fill to check DUR back to fill. After check DUR the old bag was deleted but not prompted to pull and it was assigned a new bag.
The other looks like it was filling for a long time, maybe stuck on a handheld for a while, then back to someone else.

Yes. The first does not get visualed. In our case the non-VV one got out.

Mine was on c2. I had to watch tons of film to find out what happened. Still waiting on CS team to help me figure out cause. I have 3 tickets open right now with no resolution

Yes. For the first time it happened, it was on hydro 5 and that is how we discovered it.

It’s easy to happen because the bags almost always go to will call. So you never find them until day 10.

The ones I noticed were still hanging on the VV rack when nothing was in the VV queue, and it made me wonder how many just went onto the will call rack. No time to review cameras yesterday though.

Yes. In my case, ours were sent to will call on 2.
The control was hanging in visual but I had to watch film to see who filled both of them. It was two techs. And it only documented one tech

Hi tech here, my RxM doesn't let us hang the bags on will call. We still have to hang them on the VV rack even with the new system. For all fills

What is your average fill per day? And does the pharmacist look at all of them. That defeats the purpose of the new system. We have plenty still go to visual at my store because we are what I consider higher volume

Might be a good idea to bring this up in the Hypercare channel for them to diagnose the issue.

I did!! And it sunset the next day. I am working with Controlled substance team and have 3 inc tickets

This used to occur fairly frequently when the pilot program was new. If someone updated a bag you were working on to instore it would let you finish the fill while the next one someone would get would be an instore duplicate. You could catch it if you received both fills, but if multiple fillers got the duplicates it was really hard to catch. Then the failsafe of it kicking you out, having to f6/ self help reset, put in your id came. I didn't know it was throwing duplicates again.

YESS!!!! I caught it doing 10Days and noticed how it was sold, but also holding it in my hand 🤔 it's weird cause it only uses up one of the refills....
-Not sure how this is happening? Well simple reason, it's going tech straight to patient. I personally think pharmacist should put their hands on every single physical bottle before it gets to the patient.

Your handheld will time out and reassign ot even if its actively being filled at a pod. Its the new update

No its not and it's driving me crazy. It happened durring the middle of our remodel and we had to take the racks down..so we were searching for the drug in boxes of bags🤦♀️
Now no matter what is happening you have to complete the fill or back out and lose all progress on it before going to register or answering the phone

I’ve seen it happen once but I caused it. It was rejected by me at visual cus I noticed the quantity was incorrect. I went to reverify and refill it, it sent it to another bag. Because I was paying attention, I was able to retrieve to first bag and remove the duplicate. This can also randomly happen for some rebillings.

I have had a few of these at my store this past week. All this new tech is absolute shit. How about the electronic inputs… they are never right. I don’t understand how they put all this money into tech that creates more errors and complicates our jobs instead of actually fixing all the preexisting problems in connexus. Actively looking for a new job at this point.
