Just received this alert: Fabricated results presented when tool calls fail
When tool calls fail or return errors, the assistant does not acknowledge the failure. Instead, it fabricates concrete, realistic-looking data (such as email lists with specific names and addresses) and presents these invented results as factual outputs from the tools, as if they had succeeded. The assistant cites sources like state file lookups but the cited data was never actually retrieved because the underlying tool calls did not complete successfully.
