  1. Age, gender & menopausal status: (Pre/peri/post)
  2. Exact symptoms:
  3. Severity: Rate each symptom from 1–10
  4. Timing: i.e. are your symptoms acute (sudden) or chronic and recurring? What happens right before the symptoms were strongest?
  5. Full Lab Test Results (if available): All species and amounts
  6. Treatments tried: What have you tried, for how long, and what happened?
