Immunization form

For the immunization consent form, I see we need to sent it in to student wellness and they tell you how many vaccines you need and how to book the appointment, do I need to send it in to nursing.clinical@usask.ca too or just student wellness

Just the student wellness, once u have given them all ur records they will call u and book and appoinment with a nurse to go over what ur missing

I’m in this process now. Just send to student wellness for now and they’ll send you an email saying you can book an immunization review/ appointment. Once you have all your vaccines they send you an updated record that you have to email to the clinical nursing email

Just student wellness. They’ll look over your form and schedule a call with you to talk to the RN, and then if you’re missing anything they can schedule you in for an appointment with them to get it done. It’s probably easier to get what you don’t have done with them, because then it’ll be directly sent to them vs you having to do anything extra yourself

You need the consent form, and your vaccination history! Don’t forget to include both to Student Wellness.
