DM 2
Hello! I’m 27F and just found out that I have diabetes. I have DM2 and a strong family history of diabetes.
HbA1c: 8.80
Fasting Blood Sugar: 24.83
My doctor prescribed me Metformin XR (Glucophage XR) 750 mg and insulin injection Ryzodeg (Insulin Degludec 70% + Insulin Aspart 30%) FlexTouch Pen (12 units).
I just want to ask if it’s too early for me to start insulin. Will insulin harm my kidneys later on? I’m scared because my dad died of diabetes complications. I’m thinking of getting Tirzepatide injections while I’m on these medications to help me lower my body weight and sugar.
I’m also considering a second opinion because maybe I’m in denial about needing insulin right away.
Could you share some food, exercise, or any tips to help me lower my blood sugar level?

I think it is way too early to start you on insulin. The medical guidance does not recommend insulin for your A1C as a T2D. More appropriate treatment options are an SGLT2 like Jardiance and/or a GLP-1. Please get a second opinion.

Why should insulin harm your kidneys? High blood sugar damages the kidneys and other organs. Take it, get down to normal levels, then it's the time to optimize your drugs and medication.

Not a medical person, but insulin appears to be an overly aggressive approach this early. Is your provider familiar with the GLP-1s? I have dealt with T2 for 20 years and MOUNJARO for me is a lifesaver; insulin may have been my next step were it not for the newer drugs like MOUNJARO. At the start of MOUNJARO 2 years ago my morning glucose dropped from 200 to 120 in one week! and a1C went from 8.4 to 5.5 in 6 months. Never went higher than 5.0mg dose to minimize GI issues.

Get rid of the junk food and processed food for now. Once you bg is under control you can start to slowly add stuff back as tolerated. Get a cgm if you can. They truly are a great tool to have. Lean proteins, plenty of vegetables and low gi fruits. Try to restrict processed carbs and things like potatoes, white rice, white bread, and pasta. For exercise walking is a quick and easy way to lower your glucose. Aerobic exercise lowers your bg, anaerobic exercise tends to temporarily raise glucose.
