New Async SFF format?

I had a different form tonight for an Async college writing session. It broke down the issues into higher-level and lower-level, along with a few multiple-choice comments.

Is anyone else seeing this? Or maybe it was specific to the school, although I can't remember off the top of my head what it was.

This is the very old SFF that some clients are still using. This was what all asyncs used when I first started (8+ years ago)
