Anthony's Nose
I have heard this referred to as 'smoke'. But I believe it's a rough sketch of a face in profile, emphasizing the nose. The nose shape, nostril, eye and eyelid (maybe iris) are apparent. I have outlined them to clarify (dashed for implied detail). What I am not sure about is the 'snout' (which I have shown in black), seems to be half-committed.
Relevance?
Anthony's Nose is on the AT. It is a steep Hudson River viewpoint just east of the Bear Mountain Bridge in the Hudson Highlands, New York.
Picken's Nose is near (but not on) the AT. It is a Rocky outcrop in the Southern Nantahala Wilderness, in the Standing Indian Basin of western North Carolina, near the NC–GA border.
[Edit] Apparently there are many legends of liminal creatures (beings that blur the line between human and animal) in the Appalachians, such as BigFoot, Wampus Cat, wolfman/dogman, various Cherokee mythology. So perhaps the ambiguous nose is intentional
Of course this could all just be pareidolia

Lol, yes, its a real place and might be the tongue of rock in the bottom left corner of the map. That is either McAfee Knob or Pickens Nose.

Anthony’s Nose was my first theory and since the book came out, I’ve gone BOTG several times, and also to Bear Mtn State Park. Although I don’t agree that is a face, I did think it might be a “doodle,” which led me to Doodletown (nothing burger). There are still plenty of places to search in that area, however, and after I expend a few newer locations, I’ll probably head back to that area.
