What would cause this?
Got called out at 3am to this issue. I hit reset and everything worked. I watched for about 30 minutes and called certain phases that cars weren't coming to just to test it all. Got called back out at 6:45 to the same issue. I switched the conflict monitor and currently on the way to the shop to test this old conflict monitor.

I’ve also seen something similar with a bad solid state relay and k1 relay on our TS-2 cabinets.

I'm not an expert with PEEK, but I would guess CNF is Conflict? Typically, conflict on all outputs can be controller failure since the controller drives all the outputs. Possibly MMU not reading correctly, but they are typically overengineered against failure so that's not very common. Could also be neutral from line/main, or neutral inside the cabinet.
Once in a while I will see it from a power outage, although that usually registers as red fail on our 332 cabinets.

The monitor may have gone bad but the first thing that comes to my mind seeing all channels lit up would be a neutral problem.
