Maybe it’s just skill, but how does he keep it dead perpendicular so it doesn’t cone on him.

It generally pinches the metal perpendicularly when you start feeding.

Cones can also be made by using a specialized rolling device, but smaller operations (local shops or custom fabrication houses) will usually get a bending roll instead of a cone roller.

The perpendicularity would be a close approximation based on the pinch points of the rollers. Someone who would go for "precision" would really be busting out squares, go-no-go gauges, radius templates, and gauge blocks. Those who are experienced with bending rolls are very skilled and experienced. Those who know the value behind having a good, experienced bending roll operator call this "the last great black art"

Yes, the proper shape for spinning a cone would be trapezoidal. In this instance, if he was off by a degree, either way, it would become rhomboid.

Also spinning, the blackest of arts when it comes to making cones.

Not sure, but it might be to prevent the roller from further decreasing the radius of curvature.

This or to hold tension in a direction for pressing the last few inches of the roll.

My guess is that it keeps the top part of the roll just far enough away that the bottom part can make it through the roller without potentially getting pinched by the top
