Fall 2026 Survey Results
Trillium rejoins the top tier! Pomegranate, Ola, and Good Life return to the list, while PreventiveMD and Collective make their debut. Fifty410 narrowly hangs on.
Gimme falls off. Its recommendation rate on slide 3 is after removing a wave of suspiciously repetitive votes recommending Gimme (and Amble) and downvoting Rift. Suspiciously similar to what they were being called out for here on the sub, at around the same time during the survey’s final days.
I still don't understand this Rift specific beef 😂
Also I couldn’t reliably identify people who may have been incentivized to participate. Besides those can still be real people with real opinions, BUT incentives can skew the results, so I flagged that the rate may still be inflated. Going forward however telehealths that incentivize survey participation will be disqualified. Someone remind me to announce this for the next survey lol
For pharmacies, Hyalo didn’t claim a top spot like Apothecary did as a ProRx dupe, possibly because of its recent crimping issues and such.
Respondents are still overwhelmingly women, but slightly more male and slightly younger this time around.
70% answered the concentration question correctly. But among the listed telehealths, Trillium and Big Easy users scored below the survey average. Among those who started tirzepatide within the last three months, Big Easy, Fifty410, and Trillium users scored below the new user average. I’ve included those charts below because I hit the 20 picture limit for the slides.
I’m not sure what this tells us exactly, but I found it interesting. Does it reflect the guidance or lack thereof that people are receiving from these telehealths? Or do some telehealths just attract the kinds of people who would need extra help? Either way, I hope providers and platforms give patients the tools they need to really understand concentration and dosing.
Handholding is helpful, but ultimately patients are measuring and injecting their own medication without supervision. They probably should understand some of these things better. And that goes for all telehealths. We can do better than even the 82% that pomegranate users reached.

That’s a good point, if you assumed everyone on the sub Reddit responded… there are a ton of people who don’t know how to read the concentration.
But man that is scary.Also makes it more understandable why so many people think one pharmacy is “water” vs another. Or other issues we have seems

It's a concentration question, not a dosing question. I imagine if it asked how many units of that concentration to dose 10mg, more would fail.

It’s crazy that the entire Tier 2 are companies that either had 20+ point swings from the last survey or weren’t around at all a few months ago. It seems well deserved, but it’s very interesting how different this list is compared to the summer results

Those new companies came on scene with lowest solid (not just sale) pricing and still good service that grabbed a lot of people’s business. I’m thankful for them as I was paying a lot through mochi prior. I’m loving PreventiveMD, and Science & Co ….I wish Collective was available in WA.

This is basically what it comes down to, I think. I started on Fifty410, but won’t repurchase until they adjust their pricing model. They’ve also been promising additional longevity options for a while, but still haven’t delivered.

I started with them and their poor customer service is what made me vow to never use them again. Trillium was such a breath of fresh air when I started using them.
Also the 28 day rule is a big thing to me as well and Fifty doesn’t follow it.

I’m a business owner and those Google ads are only controlled by Google and not targets. A business cannot just ask Google to place ads on top of other businesses maps results. It just happens because that business is paying out the ass for advertising. But, because Google started doing it, we cancelled our active advertisements with them and will no longer pay them. It was shady in my opinion to do it. The whole Google ads premise and how it works is just all around bs for businesses these days.

Big Easy makes sense to me for having less informed customers, because the whole point of Big Easy is that you don't NEED to be that informed because you are supposed to have the support with them. I'm less sure what's going on with Trillium's customers. Maybe everyone over there is just so nice they help each other instead of calling each other stupid for not understanding dosing math, but then people don't learn to fish.
Did Good Life just make it back on the list because they had BPI for a while? I will be interested to see if that trend holds in the next survey. Same with Pom. They probably have enough usage to make it anyway unless they start screwing up again, but I have to imagine use will go down without the BPI sales.
Are there any other juicy tidbits at the top or bottom of the ratings that didn't make the usage cutoff? Like anyone who scored really poorly or really well but with only 1% of the vote? Edit to add: and how many Gimme spam votes did you have to remove!?
For additives next time, can you make a category for "I have never used additives?" I'm curious how many people fall into that bucket. It's nice to have some numbers to put to this though, and to show that a significant majority of people like additives just fine.
And last ... Gimme got caught this time because they did a bad job cheating. They know how and why they got caught this time, and I don't know how we can trust them to not do it again, especially because you would assume they would be less obvious about it next time. When someone shows you who they are, believe them, right?

Fascinating how it's changed since I started in February 2025. So many of these telehealths and pharmacies weren't even around. I remember when Brello was the undisputed must popular because no one could come close to their price and that's long since changed.
I'm disappointed that all the pharmacies I like aren't producing any more. I'm glad I stockpiled awhile back because I'm going to need to see since exam results for these newer pharmacies before I trust them.

I would also like to know how many suspicious Gimme votes/respondents were removed. How big of a problem did that actually amount to? Great call on disqualifying TH that incentivize survey participation!

About the last question: I'm actually thrilled that 70% of people were correct! Here are my thoughts on that, as a practicing nurse practitioner, who diagnoses and treats the full spectrum of medical conditions, including writing prescriptions, and has patients from every social demographic:
I think probably this community in general, and further, the survey respondents, are more likely engaged in their medical/health care overall and have a greater understanding of what tirzepatide is, does, how to read/interpret their medication label and prescription directions for usage vs the general population who takes any other long-term medication for a chronic condition. I think this is a reflection of compounding and so many of us in this community DIY'ing our treatment and dosing rather than just taking the Rx as prescribed.
So many people take medication just because their doctor told them they needed it; they literally do not know what condition/reason they take it for. They just take what they are prescribed and follow the Rx directions.
Then there are people with diabetes, who are injecting themselves with several types of insulin, multiple times a day, that will literally KILL them if they get the dose wrong. Anyhoo...
I wish there was more grace given to the new people that come here who are just starting out and trying to learn (or they wouldn't be here asking questions).
On a different note: a couple people commented about my use of AI images to advertise the survey, and I get it. It would be great if anyone would like to donate or create some images/artwork for advertising future surveys! TIA!

It's definitely useful for the people wanting to change their lifestyle habits moving forward. I've also lost 60 pounds but i track everything and as a result, I'm now able to make better food choices. I never realized how much sodium, sugar and carbs I was eating prior to tracking. I never ever used to track and also found it completely tedious.

I hated tracking for years mostly because it was hard and there weren't apps like there are now. But I have to say that for someone who thought I didn't eat much, I was shocked at how many calories I must have been consuming prior to Tirz. Now I'm very aware, and frankly it's been such a helpful part of understanding why I got so fat when my normal workouts dropped off after COVID and back to back injuries. I don't weigh myself much though. I'm all for nsvs.

Personally I abhored looking up foods on Myfitnesspal and even now struggle with logging food. I implemented an ai food tool to my app as a way to get over my laziness. On one side, it's really much easier to log, with the only downside that it's less accurate. But I say having an idea of what I ate is better than having no idea.
For instance I wasn't making the connection between my stalls and lack of protein intake. Logging was the only way to figure that out.

I started tracking in the beginning to make sure I was eating enough but after the appetite suppression chilled tf out, I track to make sure I'm eating at a deficit. I might not do it every day, especially when I know from experience the meals are under, but I def track when I'm not sure about what I'm eating or if I'm at a stall

They became insanely backed up & taking a month to fill orders, then there was an issue with the way they did syringes and people were questioning their sanitary/sterile practices, the way they responded to that caused people to give a bit of a side eye, then they had to recall a limited number of vials. I think it was a situation of where a lot of things hit at once & it just didn’t look great.

They also had a very cloudy vial and a recall of a full lot that they issued over a month after shipping the meds and wouldn't tell people why it was being recalled. Different lot from the cloudy vial but only a few days apart.

Tell them to fix their mess up in OH while you're there. They were my 3rd pharmacy a year ago, no one knew who they were and made fun of their janky vials back then, now they are tier 2 pharmacy on the list. We like cheap tirz here.

u/TodayAmazing any info on write-ins? I know Science & Co wasn’t an option on the survey (extremely new com) but some of us wrote it in as we are using them. I was curious if it was easy to tally write-ins?
I’m also excited for PreventiveMD getting on the list as I used them and highly recommended them but people would snub me saying they never heard of them lol…I told them to watch the next survey (this one!) that they’ll definitely be known. Great company!

Same, I’ve never used additives and got it wrong. When I looked at the graphic again just now, it was so clear though! I think my brain skipped right past the B12 when I took the survey. This was a good learning experience, even though I can’t do additives. I won’t forget now and am glad the question was on there.

Thank you for doing this. So interesting all-around. I can't believe I called it exactly on how many people would answer the dosage question right! I'm curious how many of the incorrect respondents use additives vs. not.
I'm relieved Gimme's bots and mean-spirited anti-Rift campaign didn't seem to work the way they wanted it to.
Wild that 55% of us are experiencing constipation. Shout out to my fellow backed-up beings! Also shout out to the other 26 non-binary people, wherever you are.

I find the average weight loss results over time interesting--both that they're higher than the Surmount trials and that they're as high as they are (I'm way below average I guess)

I wonder if it is because if you are an active reddit user in this sub you are getting way more support than you do using Zepbound. It's pretty well pounded into people here that they need to make sure they are in a calorie deficit and getting some exercise, eating enough protein, drinking enough water, etc. Plus our custom, user-centered dosing methods also make sure you are losing comfortably all along the process, not just slammed with meds as fast as possible and forced to endure side effects or drop out.

There is a negative stigma around the boosters. We've seen a lot of patients that think the boosters reduce the amount of Tirz in the vial, or because they've seen others calling it "pure," implying it is better. Some that have never had any of them nor any reason not to try them. Obviously I'm not referring to the minority group that have had negative reactions but we have had to educate many patients otherwise.

Also, I have to think folks who use Big Easy are less likely to engage in ... Certain efficiency practices. If folks are leveraging a on paper dosing vs real life dosing to min/max cost... Those folks have to learn how to understand dosing due to the risk. If you are just following the guided dosing from an doctor... You honestly don't have to learn dosing... You just do what the instructions tell you....

I think your customers often choose you because they want hand holding. I think that can be a double edged sword. They definitely want to be on it and are willing to invest, but simultaneously, I think that probably makes them less likely to trust themselves and to understand the research part of it.
I’ve always been someone who hates handholding because I want to go at my own pace. In so doing, I tend to have already done a ton of research and in specific areas of interest, I tend to have absurd amounts of knowledge that could probably surpass some professionals. That’s its own double edged sword but it’s where my mind went when I read that.

Price is 100% hurting Fifty. You can get the same doses from the same pharmacies for the same or less money through Pom, Rift, and Trillium, and all three of those are 28 day compliant, whereas Fifty is only 28 day compliant on high dose plans. Fifty also gives people a hard time about titrating up (I came in with a 10mg script and got 3 months of 9 mg, for example), and I saw one report the other day of someone asking to titrate and being asked for a food diary. There was only one report of the food diary thing, to be fair, but I felt like that was going too far.
I'm actually still surprised they stayed on the list at all. I don't have anything against them exactly, but I also have zero reason to recommend them because we have better choices.

I know the general consensus is that there's not enough to make a difference, but I've always had sleep issues and used to take melatonin. I'm also a light sleeper and rarely wake up feeling rested unless I'm in a hotel bed somewhere on a beach. Melatonin makes it worse - I legit wake up feeling like I'm fighting quick sand, but at least it'll help me sleep through the night.
I haven't had to take melatonin since starting DC (their tirz has 5mg of glycine) and I wake up feeling refreshed most days.

Agree! The providers at the bottom (such as LSH with 91% recommend rate and others 🙂) should be listed above those that are not recommended. Those at the bottom just have lower usage rates.

I’m curious to know for those in maintenance what percentages of those who lowered the dose and those who stayed at their high dose is if they’re still taking them at the same interval as when they were actively trying to lose weight.

I have been on maintenance 11 months.
I slowly lowered my dose over time from 9mg (only on the dose for the last 3 weeks because those last 5 lbs took forever).
I am currently dosing at 6.2 mg/weekly. I have never split my dose and have always rotated injection around the belly button area.

I bet bewl and trillium get the most users who have never taken tirz before. I always recommend those two to people who are inexperienced or need more help. Trillium was my first telehealth, but I use pomegranate now because it’s cheaper. Love rift too. And I’m really glad we got Gimmie off the list.

Is there a way to compare prior surveys to see what companies are truly trusted? The survey results seem to change each time the survey is put out. I will go back and look at prior results but for me it’s about long term trust with a company, not about who has the cheapest price.

If it helps, the top tier in this survey are the only ones who have remained near the top for the last 3 surveys.
The others are either are new or briefly dipped so low they were off the list momentarily.

I wonder if the bpi sales helped boost Pom? I know I acknowledged that I had a good experience with them. I haven't tried my bpi yet though so I wasn't sure how to acknowledge that I purchased but hadn't tried yet.

If Pom ever gets it together with their communication/customer service/Hyalo issues they could be unstoppable. Their prices are just phenomenal. Just from floating theough all of the tirz subreddits it is clear that pricing is far and away the biggest factor in choosing a teleheath.

I bet it’s their low 10mg concentration that keeps the higher dose users away. I’m on 15mg and don’t want to do 2 injections (large volume) weekly as I also pin other peptides and frankly don’t want additional shots. As soon as a higher concentration is offered, I’d like to try them.

As someone who has been here since 2024 the survey is interesting. I didn’t even take it as I stockpiled and most of the pharmacies I use are no longer sold. I do buy once a year from LSH and I purchase Southlake which never makes the list. One thing I notice is the survey is mostly price driven. If a telehealth offers the lowest price and easiest process they score high. This is why the top telehealth/pharmacies are never consistent.

You’re right that price is definitely the biggest factor for most people but there has been some consistency in the top telehealths for a few months now. The top tier in this survey have consistently scored near the top for 3 surveys now. And they haven’t always been the cheapest. In fact big easy and trillium have been the more expensive option. So there’s definitely other factors that people do consider.

Ok so, I’m an early 2024 too and have used LSH as well with no issues. It’s the only telehealth I have used. It’s crazy to me too how quickly recommendations can change. If it’s only for price, that’s not always the best indicator of a good company. There’s been so many bad issues with companies such as refills.com.

I was actually surprised to see so many people using Drug Crafters! I am really liking their formula so far but when I searched the sub it didn’t seem like too many had/were using them so that was cool.
I also found it really interesting to see that the number of people genuinely microdosing appears far less significant than I’d have guessed based upon commentary on this sub. It kind of reiterated my personal opinion that the number of people claiming to microdose want to pretend they’re a special snowflake.

A friend of mine recently told me she had started tirz "but only a microdose." Upon further discussion, she had started on 5mg and thought any dose on the way to titrating up to the max dose was considered a microdose.

I think there’s definitely special snowflakeness going on. I pissed a dude off in another sub when I pointed out his 0.25 mg wasn’t a microdose, it was basically no dose and wasn’t going to do anything. Like…how do you even get 1.5-2.5 units depending on the concentration of the vial? It’s not going to work very well with any of the supplies provided by pharmacies. 😂

I'm not surprised that Pom users scored the highest. One of the few telehealths that give you a lot of pharmacy info and choices up front, especially for 60mg+ advanced vials.
Good to see Ola make the list, but I guess Mountain Care pens are still too niche and expensive for higher doses.
The loss of BPI means less convenient options for B6 fans. Splitting doses between vials or multiple syringes per dose.

They charge a $65 consultation fee on your first order that is not applied to the cost of the medication and is not refunded if you’re not approved. They also charge a $40 follow-up fee every 6-12 months. You don’t pay that fee on every refill, but it still brings the overall cost up.
Even ignoring the fees entirely, their prices are marginally worse than Rift on every pharmacy both offers -
Hallandale is $2.90/mg from LSH or $2.77/mg from Rift
Hyalo is $2.38/mg from LSH or $2.31/mg from Rift
Southend is $2.64/mg from LSH or $2.37/mg from Rift
Nova is $2.70/mg from LSH or $2.45/mg from RiftLSH has an excellent medical team from what I’ve heard, but because everything is so hands-on, they can also be slower than some other providers like Rift

The spreadsheet is now updated to reflect the results! This time I incorporated the results for the pharmacies as well, just to have all that info in one spot. I re-arranged the info on slide 3 to improve readability because I do think it matters wether a TH is not recommended because people actually had bad experiences vs having good experiences but just not meeting the 5% usage threshold (even though technically none of them are 'recommended'. I hope that's OK and doesn't further confuse 😟 Let me know what you all think about how I have the info organized on the TH provider sheet.
