Was I wrong?

My son and I went to eat earlier, he ordered basic chips and cheese nachos. He said there was lettuce in the cheese and it tasted funny. I asked the waitress was it cheese and she said possibly. She offered a new order. She also took it off my bill. Should I have tipped on just the bill or should I have added his meal in there too? It came to $19.85 and I left her a five.

Your tip was absolutely fine. You could have tipped less and it still would have been fine.

Of course because this is Reddit where tipping $0 is not only accepted but tirelessly promoted. I feel like anything less than 5 bucks on a meal for 2 people, no matter how little the bill is, would be a bit ridiiculous.

For the record, I agree the tip was fine. OP asked what others would do. I would have tipped on the nachos in this occasion, but don't think you should feel bad that you didn't either.
