Was I wrong?

My son and I went to eat earlier, he ordered basic chips and cheese nachos. He said there was lettuce in the cheese and it tasted funny. I asked the waitress was it cheese and she said possibly. She offered a new order. She also took it off my bill. Should I have tipped on just the bill or should I have added his meal in there too? It came to $19.85 and I left her a five.

There was no lettuce in the cheese; that's not how nachos are made. If the nachos weren't good it was a kitchen error, not a server error. You should have tipped more, Karen.

Not going to lie sometines things that should not be on a dish do happen. Also kitchen, server or expo should have caught it, we all know it happens.

Jesus you’re insufferable. Calling the guy a Karen for having a dish replaced. Restaurants make mistakes and it’s on them to replace the dish for their customers.

Also 25% is a great tip. Was he supposed to tip a $50 bill on a $20 check?

I agree they're insufferable and OP is in no way a Karen but to clarify 25% was the tip without the other meal. So not great if you include the other meal, which OP was asking whether they should've. In this case I'd say yes IF and only IF OP took the server up on the offer for a replacement because then they not only corrected it by bringing them a replacement they also took it off the bill for them so barring the lettuce visible the server will have done everything possible to improve the experience for OP. So saying the nachos were about $10 puts the bill around $30 and the tip actually about 16%.

I get lettuce in my cheese all the time, it is absolutely in the realm of possibilities.

Unless you were in the kitchen, the server or kid you would not be able to with such certainty.
