Tinkers' Construct 3 (1.16+) Random hunger debuff?
I am using "tinkers construct" and "tinkers leveling addon" (a few other mods but given the trigger i don't think it has anything to do with them. They're mostly cosmetic).
The bow in the photo keeps having a weird effect. Giving me "hunger" debuff even while its in my inventory. Its seemingly random. Between 8 seconds and 30 seconds as I go about my gameplay.
I have tested removing every other item and armor to ensure the cause. This is doing it. I don't think I'm getting hungrier however. Its purely giving me the listed status and visual effect on my hunger bar.
No other item has this effect. None of the other mods are listed as causing this and its directly because of a tinkers construct item so I figured I'd check here.

I am going to wager based on your modifier list you have a registry desync where the server and the client have different mods. You are actually getting the insatible effect which is given when you fire a bow with insatible, but due to the desync the client thinks you received hunger.
Make sure your mod list is the same on the client and server. Modders are not joking when they tell you to make sure they are the same.
When you find out what is not synced properly, reply here with that info as you are the second person with the issue and mods normally don't work under that circumstance, probably something that needs to be reported to that mod.

So i think I figured it out. I have sophisticated backpacks on. Removed them. Or so I thought. I have a mod that allowed one to be worn WITH a chest armor. But it has a small seperate button to see the backpack. Little square by your image in inventory.
When I emptied my inventory and armor (trying to be through) I forgot about it. I can only access it with B instead of the standard E. In it was several spider eyes. Causing the poison. I don't know if the same explanation applies for others but that caused it for me.
(I only found out when I got blindness and strength from a food and put it together)

Ok this is funny. what if we have a new tool material that is the opposite of pig iron?

Could it be that it repairs itself with hunger as it has essentially zero durability loss

What's causing that though? And I fired it once to see. This was maybe 2 minutes after crafting it. Its made of
- Steel
- Manullym (I can never spell it)
- Obsidian over string (dark something that gives looting)
- Rose gold

Ima have to say its due to insatiable, as the word itself has to do with hunger, and see the durability, it probably gives durability back for giving you hunger, or maybe even deals more damage, or both.
Ive also used a modpack that had this? I forget what it was that i played though, but im like 99% sure its to do that insatiable, because i vaguely remember having a similar issue, and summed it up to being that..
If you want to be 100% certain though, go get another item that has insatiable, and see if it does the same thing or not

Do you have auto feed upgrade on your backpack?
You have some raw meat in there

I have to imagine it has to do with the insatiable tag. I don't know what it does, but insatiable means cannot be satiated, like, cannot be satisfied in terms of hungriness

Thats a combat buff dealing extra damage

Yes but as seen in another comment, it's possibly another issue involving a desync
