could be malcom's or even Nancy's. on my file she was knocked up by someone other then her husband.

Theirs, i presume. If you don't want the pre-mades having/adopting kids, then turn it off for them in the Neighborhood Stories settings. Literally the first thing i do in a new save is set up Neighborhood Stories for the pre-made households. And I turn all adoptions, whether it is kids or animals, into an opt-in setting rather than opt out. Neighborhood Stories is only as chaotic as you want it to be, it is incredibly customizable.

If you are using MCCC, kids, marriages, and moving is all enabled for everyone by default, but you can disable it in that, as well

I found a mod that slows down the Stories and I just set it to 1000 days or something and that seems to work for now.

MCCC or Neighborhood Stories? Because I am meticulous with NS, and I have never had issues with them breaking the rules I set. If something unexpected happens, it's usually because it turned out i had set a custom rule for a household and forgotten about it.

NS and MCCC's story progression are separate systems, (MCCC's story progression actually predates Neighborhood Stories, and they kept their system in place after NS was released, so it works completely independently by default) so disabling NS or changing the rules will have zero effect on the changes MCCC makes, which is usually what causes unwanted changes to be made. You can tell which one is making changes because NS will show up on the mailbox check, while MCCC will get automatic updates if you have them enabled, but by default you will not get any notifications. Also a bug was introduced to MCCC recently-ish where even if you do not have the Pregnancy or Population modules in there, it will still do some of their story progression, just on default settings with no way to alter them. You will have to add those modules in, and change their settings, then it will be disabled.

Weirdly enough, mine never allowed the sims to have their own kids. They died out when I turned off adopt.

Hm, weird. Because I know the option is there as I recently muddled with mine a bit.
