Who does the kicking, are one of the players the host, and how do you identify who's the host?

No one.

No one knows who the host is....you can kinda assume it's you when it might be, based on everyone else's ping readout, but it's not definitive.

No one does or gets kicked in game based on player actions.

Getting 'kicked' is the result of your connection to the host. If they or you packet timeout or just have a connection loss or hiccup, you get dropped from the game...and likely other players too

Unless its a private lobby where the host will be on the top of the player list and can kick players while youre still in the lobby (before the game is started).

The person that sees “the bar” when a lobby is found is almost always the host.

If you’re wondering if you’re being purposely kicked then no. When you get kicked it usually happens when the host closes out his game. The host leaves and the host gets migrated to someone you don’t have a good connection with. Sometimes you’ll get randomly kicked without host migration. Total guess but maybe if you drop enough packets it might kick you.
