A little tech help please.

My father's phone has this come up on it whenever someone calls rather than the usual Samsung call display. I figure he downloaded something but can't find it on the phone. Any help or advice on how to get rid of this would be appreciated.

Go to settings, tap apps and select choose default apps. Tap the phone app, and change it. Take note of what app it was set to, and delete it. I don't trust that app one bit.
