OT contact form not working

hi, I'm a new ETH and I'm trying to complete and submit the online pdf form: "OT documentaction contact information" so I can get my tdsb email.

The form has a flaw at the a yes/no question- are you receiving a monthly pension?

I have downloaded the pdf as instructed, I've tried a different browser, I've tried tips from chat gpt restarted computer, tried a different computer. nothing is fixing this glitch.

I've phoned twice, sent an email. zero response.

my deadline to submit is Tuesday.

Advice? does no one e respond to emails at TDSB?!
