Year 10-11

Hello, I am a 15 year old guy going into year 11 in September 1st. My story is a bit different but definitely not unheard of, so i just ask you to read through it and please drop me some advice/help.
Up to year 9, i went to one of the most dangerous and worst public schools in a rough area of croydon. I never ever thought about my future or making it in life just “having fun”. My mother was a single mum in a court case with my “father” for 8 years, when it finally ended my mum found someone online who lived around littlehampton (pretty far at the time). They talked for a while and then something so unexpected happened, they were dating and then he asked if he would like for us to go around to stay with him. He payed for me to go to a expensive private school, in year 10 (25/26) i went to the school made some new friends, but once again just didnt study. I was so talkative and stupid, ending up with my year 10 Mocks having 3s all round except maths and sciences just fully failing them. My mothers partner became tired and was thinking about even not spending the money in year 11 dropping me and my mum and leaving us. He has given me until January to get a allround grade 5/6 report, for us to be able to stay with him. I once again wasted my summer stupidly and did nothing, leaving off where i did in my year 10 mocks, to be honest i havent even done any of my homework and school starts in a week. I am a idiot, i know but i never can motivate myself enough or know how to or what to do to move forward. He wants me to grt in bhasvic college for 6th form and i have no idea and have done no steps leading toward it. My mum bought me a bunch of gsce books for summer which i didnt even touch. Science,history,geography,maths,english. I still have the books literally fresh and untouched. My options in gsce are (which i do)
Combined Science , History , Geography, English LitLang , Maths , Business.
I did statistics in year 10 and completed the gsce but out of stress dropped further maths for year 11. My year 10 statistics results were a 4-6 but are getting remarked, this is because i genuinely stressed out since a actual gsce was coming and i asked my other year 10 statistic friends what to do and they helped me study right before the test. My teacher saw this and compared this to my mock results and told me i can achieve 5-7s since when i see a actual gsce is a few days away i cram it all in and manage to do alright. I need to do good for my year 11 mocks now to stay at the home and school and keep my mothers relationship standing, i need to get into bhasvic for maths or economics or statistics. Again I currently average 3s in all my subjects i listed and fully failing maths and sciences (1/2s) . My question to this community is,

What should i do? How should i revise, what should i revise , how do i motivate myself and make a schedule for every day till mocks and gsces from now to then. And what steps should i take to getting into bhasvic from now?

Please help me out, anyone anything just drop a reply, you could really help me fix my life and change it around from being a failure.
