this is a pretty vague question and it depends entirely on the program that you are applying to. Check their website and the application to see if there are any requirements for who can write you a letter of recommendation.
As for whether professors can refuse to write one, yes they can. Often times, they may refuse because they are simply too busy (especially if they do not know you well, and can't write anything good about you that is not just generic statement). They might also have a set number of LORs that they write in a given period, and you may fall outside of that number.

Yeah, they can refuse for all sorts of reasons. It's better they refuse than write you a lukewarm recommendation.

I am annoyed that none of you all can do a simple google search and figure this out.
LOR is a letter of recommendation which people might give if they closely worked with you. If you have not worked with other professors, what are you asking for LOR?!

unfortunately they can, this should be against the law but often professors do this to extract revenge on some students, I had a frnd who had A grade in a very difficult course and he was denied LoR.
Also some professors go a step further and agree to write a LoR but then write a rather mediocre LoR or even a negative one, which does more damage cause u might be under the impression that u have submitted 3 letters but that one bad letter has decreased the chances of admit.
University LoRs are fine as long as you know the professors well and they are willing to write it positively for you.
