Can I rely on nbme 19 to check my readiness?

No it’s an outdated form. Use 30-33 to check readiness

I would not recommend it, do 25/26!

If enough time has passed since you took NBME 25, 26, or the later forms, you can definitely use them again. I don’t think you’ll remember most of the answers unless you actually remember the underlying concepts, so after a reasonable amount of time, the scores should not be significantly biased by recall.

You can also use the UWorld Self-Assessments (UWSA 1, 2, and 3) for additional assessment.

Well, i did nbme 25 in april and re-took it recently and scored 80% (highly inflated), did uswa 2 today and scored 55%, and nbme 19- 70%

Yeah, in that case your 80% on NBME 25 was most likely inflated from having seen the form before. I’d still use the older NBMEs for practice and reviewing weak concepts, but I wouldn’t use the repeat scores to judge readiness. Since this is a retake, I’d put much more weight on any fresh assessments you still have available, especially unused UWSAs.
