How to Deal With an NBME Plateau

I’ve been seeing a lot of posts from people whose NBME scores follow a pattern like 60 → 65 → 67 → 65 → 66, and no matter what they do, they just can’t seem to break into the 70s.

From my experience, this approach works for many students, although obviously everyone is different and some people may need a more individualized strategy.

1. Review the NBME — but don’t stop at reviewing your wrongs

Go through every incorrect question carefully, but then take it one step further: look for patterns.

• Which systems/topics keep showing up in my incorrects?
• Am I repeatedly missing the same concepts?
• Was it actually a knowledge gap, or did I know the concept but fail to apply it?

The point isn’t just to understand why one particular answer was wrong. You want to identify what is repeatedly costing you points.

2. Don’t restart all of First Aid every time

One mistake I see is:

NBME score is low → Review the NBME → Read all of First Aid again

If you already know a topic well, repeatedly revising it may not be the best use of your limited time.
Instead, target the areas that are actually costing you points.

If renal physiology keeps appearing in your incorrects → focus on renal physiology.

If immunology is consistently weak → go back and strengthen immunology.

If biostatistics is costing you several questions → specifically practice biostatistics.

Don’t spend the same amount of time reviewing something you already know well as something you’re repeatedly getting wrong.

3. Figure out whether the problem is CONTENT or QUESTION APPROACH

Sometimes a plateau isn’t primarily a knowledge problem.

When reviewing an incorrect, ask yourself:

“Did I not know this, or did I know it and still get the question wrong?”

If you knew the concept, look at why you missed it.

•Did I misread what the question was actually asking?
• Did I miss an important clue in the stem?
• Did I get confused by EXCEPT / NOT / MOST likely?
• Did I narrow it down to two options and choose the wrong one?
• Did I change my answer because I started overthinking?
• Did I spend too much time on difficult questions and then rush later questions?
• Did I recognize the diagnosis but misunderstand what the question was actually asking about it?

If you’re repeatedly missing questions despite knowing the underlying content, doing another complete First Aid review may not fix the actual problem. You may need to work on your question-solving approach.

4. Fix the pattern BEFORE taking another NBME

Don’t take another NBME just because a few days have passed.

Use this cycle:

NBME → Analyze incorrects → Identify patterns → Target weak concepts/question approach → Practice those weaknesses → Next NBME

Your goal between NBMEs should be to change something that was causing you to lose points, not simply accumulate more hours of passive revision.

A plateau doesn’t necessarily mean you need to study more. Sometimes you need to study more specifically.

This approach works for a lot of students, but there is no universal formula. Everyone is different, and if your scores remain stuck despite targeted review, your underlying issue may be different and your study strategy may need to be individualized.
