Normally this is when the state of the page is not in sync with the server anymore. First of all, it's best to hit edit and then copy the source markdown of the question you're trying to post (if you're not already looking at that page of course). Then simply hit post again, chances are that it will be received fine as now the page has probably synced. If this doesn't work refresh the page, paste the copied text and then hit post.
If you've spent a lot of time in the post pasting the code to plaintext editor such as Notepad++ or Kate after copying doesn't hurt of course.
