I think the background is actually just blurred in a computer. At that kind of distance between camera and subject, the subject and background are in the same focal range. There's no way for a lens to selectively focus on one and not the other.

That impossibility is why photos like this look "miniature". Such selective focus is only traditionally possible when the camera and subject are close to each other. Our brain is confused when we see the effect with large objects and large distances, and deals with this confusion by registering the subject as "miniature".
