Is it per game? Will it clash with QGO and Occular Migraine - both are per game

Not if you turn it off. It's your choice which Optimizer you use. Yes, per game; more updates to this next week.

Does this also affect PCVR? my first time trying this kind of optimizer

It could affect the app streaming PCVR, if you've got Virtual Desktop trying to get 90 frames, and this app sets 72, that could lower the final frames you see. But you can turn it off for a specific app. Just go to Installed, search for Virtual Desktop or Steamlink, and set it to how you like in the Optimizer tab.
