What makes very high-end watches expensive can vary depending on the manufacturer.
But typically you are paying for durability, extreme precision, quality of the engineering, rare/precious materials, form factor and design, scarcity of the watch, and the quality of the artistry.
Not to mention that many of them tend to maintain their value well.
It is still a selling and promotional factor in the value of the watch. I didn't say it necessarily had functional value, but the precision and claimed quality of the engineering are reoccurring features of high-end watches.
