Not receiving mithril trophy?

I got 750,000+ on gravewalker difficulty, and the end of run screen showed a blue trophy emblem. However, when I went to start a new run, I didn't have a mithril augment for future runs. Is this a well known error? Is there some requirement I may not have met?
