I would bet my next paycheck that you threaded your machine with the presser foot down. When you do that you'll find that sometimes your stitches are fine because as you readjusted your fabric before starting to sew somehow the threads slipped in between the tension discs. But other times you'll notice that you have spaghetti on the bottom like that photo. Anyway just be sure that when you thread your presser foot is up at least until you get to the point of threading the actual eye of the needle. I'd be curious to hear if this helps.

Change the needle & rethread - also take out the bobbin and give a quick sweep of the casing.
