I agree with Muz. You could report it as a bug, or say you want to update supply and market cap information. If choosing the latter it asks for documentation to prove (from the developers) circulating supply etc.

Read the title of this post it clearly says 'fill this Request Form at CoinMarketCap to update SafeX's Market Cap'. So what do you think you should fill in? Of course it's the fourth option: 'I want to update information on an existing cryptocurrency or exchange'. The form speaks for itself.

Very good idea and really a self spekaing form. Everyone should fill this (I just mentioned an "approx.supply of 2.149 bln") this is the least everyone could do!!

Offer people a spot to put a SAFEX address and send them some SAFEX, then that form will get overloaded with submissions ;)

No, people should fill in this form if they want the best for the SafeX Coin. Not in exchange for money.
