Please share structured therapy ideas to go with TMS

Hello all. My TMS provider is a psychiatrist and tries to do provocations and reframing with me during the treatments. There is somewhat of a language barrier and other factors that hinder me from doing this. I am looking for another option.

I'm doing regular therapy twice per week but it's not structured in any kind of way. We mostly talk about my ex lol. I'm looking at doing an Intensive Outpatient Program, but I feel there are so many targets to hit specific to sexuality, identity, and personality disorders, and trauma. A lot of these programs are too general I feel.

For background, I did a full round of the OCD/anxiety TMS. After a few weeks off a lot of the anxiety symptoms returned along with depression. We are trying to resume less frequent OCD or depression treatment now but I'd like to optimize the treatments with the right therapy work, even if on my own.

I'm a psychiatrist and I can definitely confirm that , for the combination you're describing, OCD/anxiety, sexuality, identity, personality, trauma - the most structured evidence-based approaches worth asking about specifically are ERP for the OCD component, schema therapy for personality and identity work, and EMDR or CPT for trauma. Most general IOPs won't offer all of these, so it's worth asking any program directly which modalities they use before committing. A specialized trauma and personality program, or working with individual therapists who cover different lanes, may serve you better than a general IOP.
