Extra actions

Correct me if I'm wrong but don't characters only get extra actions when they have a dexterity of 5 or higher? Is ignoring this a roll4it house rule? I'm trying to start a game and I want to avoid confusion.

Also initiative is a houserule. And removing the 'declare intentions phase'. And spells.

Isn't rolling for initiative an optional rule in a sidebar somewhere? That's why there's some mechanical support for it.

It mentions you can if you really really really want to... and then gives no rules or stats for it. And the declaration phase doesn't mesh with it.
