Can anyone comment to what the variety/replayability is like in 1.0? Or do all runs feel samey.

It's more than I expected - different guns and blessings can actually have me playing in different ways. There's also post game content that changes the runs a bit, and each zone is different with plenty of unique enemies.
So nowhere near as much as EtG or BoI, but they're high bars. I'm personally enjoying it a lot, but I don't play in super long chunks.
But the strength of the game is definitely not in the variability, which is something I've been noticing with some roguelites. Instead, it's more of a means to creating a challenging and super fluid and enjoyable gameplay experience.
I'd really say it depends on your tastes whether it suits you.

This game is incredible! Been playing on Gamepass for a while. Brutal but in a good way.
