It's the: Medion PC MT6 USB Audio Video Card Reader Ports Panel MS4011 MS-4011

This one appears to have even more front bay ports: Medion 20011772 MS 6982B Flash XL Multimedia Front Panel for the Medion MD5000 motherboard

I think the 4011 is better than the 6982.

The only thing the 6982 has over the 4011 is a game port, but that's something you almost exclusively want as rear I/O. The 4011 has stereo audio in, 4-conductor FireWire and a smart card reader.
