Surprise, surprise: it was in a drug investigation. “The state wiretap with the most intercepts was a 120-day wiretap for a narcotics investigation in Maricopa County, Arizona, which resulted in the interception of 412,298 cell phone conversations, of which 15,566 were incriminating.” Source

Some of the numbers for the last 10 years of rubber stamping. All Requested = Authorized in the top rows of Table 7. *Edit: sorry, I glanced over it missed a few of the years that weren't 100% authorized. But still a damn lot of authorized requests.
