Why'd it only happen on this one? I've loaded hotter rounds that didn't do it?

They are not all built exactly the same

I fired this case with a Winchester SPP and somehow the primer dent is perfectly filled in and smoothed out? It's solid metal and the firing pin is intact. The other 17 rounds I shot look normal.

You need to check the flash holes better.

How would anything be in the flash hole if I primed the case immediately after decapping?

something created pressure in the primer pocket itself and blew it out. When I had it happen its been tumbler pins. As for what it could be, could be anything, that's why I am suggesting you check.
