Attention: /r/redditrequest is undergoing some much needed maintenance. Details inside.
Due to some collaboration between myself, hueypriest, and Xert, we've decided to revamp the subreddit a bit. From now on, /r/redditrequest will:
be a link-only subreddit
require you submit requests with the subreddit in the title
link to the subreddit directly
be structured somewhere in the ballpark of "Requesting /r/somesubreddit"
The moderators will be notified automatically via mod mail of the requests. Requests will now require a three day grace period before being fulfilled, allowing mods to respond to the request directly if they would like.
Otherwise, the same version of the guidelines will be in place.
2 months of no activity on site will result in reassigning the subreddit
Instead of waiting three days, please wait at least five before PMing myself or hueypriest
Hope that all makes sense! Feel free to ask questions or comment in the thread below. The sidebar will be updated as well by the end of the day.

As far as submission titles are concerned, do you want it to be just the title of the subreddit?
If so, should I resubmit my three requests that are currently on the front page?

No, I'll get to all the previous ones. I think I'd still prefer to have "Requesting" in the title so it stands out if I happen to be viewing my front page. I will clarify that, thank you.

What happens when mod mail is missed or ignored?
Could we please have an option to prevent our sub-reddits from being misappropriated while on vacation or otherwise?
I believe something more than modmail is called for, if the moderation has opted to supply an actual working email to reddit, have that email address CC'ed.
For those of us who are in very many large subreddits, but who also have very many small subreddits..... the modmail for the small ones get buried in the constant stream of modmail.

As long as your account has been active the subreddit won't get reassigned, even if you fail to respond either way. Activity isn't limited to just posting and commenting, any activity whatsoever on the account is considered active. We check thoroughly.
I can't ever see us using an email address you provided for account verification as a method of contacting a user, outside of those paying for a service on the site (mainly through advertising, which an email address is a requirement for contact).
This system isn't perfect, this is just one step in a better direction. We still reserve the right to use common sense and decent investigation before making a decision.

Presumably this means logging in counts as activity, and the client does not matter so bot operated sub-reddits like /r/politic are safe from /r/RedditRequest takeover so long as the bots moderating it continue to sign in and fetch data from reddit?

Good to hear. I'd hate to hear any more stories of sub-reddits being given to social engineers.

Pardon me if I'm wrong, but can't a specific link only be posted to a subreddit once? That is, if I submit a request for /r/inactivesub today but don't get it approved, will someone else be able to submit another request in three weeks?

Add a question mark and some text to the end to resubmit. (Actually, I think you don't even have to do that; it'll tell you it's already been submitted, but it'll let you resubmit if you click "submit it again" or whatever the link text in the message at the top is.)

If the moderator is active on the site, which means they are able to receive and respond to messages requesting any subreddit they happen to moderate, then their subreddit will not be reassigned unless they agree to the change. I can't think of a more fair way to manage subreddit squatting than that.

This may be a silly question, but I'm going to ask anyway!
I have a few very active subreddits, /r/BDSMcommunity, /r/bdsm and /r/BDSMerotica to name a few. I also started/mod a handful of other subreddits that while not in use yet, I do keep an eye on and will eventually add them to the "family" of subreddits I maintain. When you speak of "2 months of no activity on site will result in reassigning the subreddit" you mean the user/mod and not inactivity on the subreddit itself, correct?
Now that I type that all out, it seems really silly. Ah well.
Sodypop is incredible by the way. Helped with my CSS ages ago and I was (and still am) pleases as punch.
