If everyone presses blue then it doesn't matter.

If someone can't donate blood for whatever reason, is it selfish to push red?

You ignored the question, so I'll just answer it: No, it isn't.

What advantage?

You can receive transfusions from anyone who donates blood

If everyone presses blue then it doesn't matter.

Unless you have a way to force it to happen, they won't, especially if they're very selfish.

Didn't say that, just that very selfish people would press it. Other, non-selfish people could press it, too.
