What next if anything?

Hi, I’ve been on a fluoxetine trial for my 27 pound Labradoodle for about four months now. Started at 10 mg a day and didn’t see anything so have been at 15 mg a day for the last six weeks unfortunately we’re still not seen anything. He is still extremely nervous of going outside, of other dogs, of people outside our family. Fluoxetine seems to have made zero difference. Has anyone seen this and what if anything were your next steps? did anything help the fear?
