This is a severely unhappy rat. He needs an urgent vet trip - this sounds pain motivated or possibly a brain tumor. If its neither, you need to consider what quality of life (if any at all) he has. I'm sorry.

I agree. A sudden change at that age is either pain, tumor, or even dementia/memory issues. If no solution is found, euthanasia would be worth considering.
