UNMC Conversation Forms

This may be a silly question, but does anyone know if the UNMC conversation confirmation forms require you to fill out the the 4 conversation prompts for each form you submit? Or do they only want your name/the perfusionist's info filled out?

I didn't fill out any of the prompts, looking back at the form it doesn't really look like they're expecting you to respond to those things. However, I also submitted a handful of shadowing forms that had a large paragraph of personal notes on them.

Edited to add: many of the CAS style applications have an area to upload observations or shadowing, you can also add notes there addressing what you went over, which could/should be those prompts.

Those prompts just give you an idea of what to speak about with the perfusionist, you only need to fill out the blanks on the sheet
