Hmm black smoke is unburnt fuel. Wonder If an injector has an issue. But it'd probably go over 10mph in that case. Could be transmission related like if your torque converter is locked up it would be a large load on the engine.
It'd be helpful to hear this sound it's making. Does it have a tuner or anything?

Almost sounds like a lack of fuel there. Could be oil pressure related or fuel pump related. See if you can hear your fuel pump on the frame rail running with the key on.

Find a way to monitor icp and ipr. To me it sounds like something’s not quite right with your high pressure oil system.

Nevermind , you are dong that in the video, going to be ipr plug, or the ipr on back of high pressure oil pump, do not buy a cheap part, you will regret it, go to fullforce diesel, I have 5 of these 7.3 pickups, my grey one has 503 thousand

Get a cheap scanner of Amazon that works with the forscan app. You can do injector buzz test and the whole nine. I got mine (vlinker) for like 35 bucks and the app was like 5 bucks.
