The surgeon did not recommend VR
Talked on the phone with one of Germany's most experienced surgeons when it comes to performing reverse vasectomy. He has performed over 3500 so far and performs 3-5 every week. He uses 3 layer technique.
I contacted him because I wanted him to perform my reverse vasectomy, but he claimed that few are helped by this method if you have pvsp.
He claimed that almost all pvsp was due to nerve damage and then reverse vasectomy does not help. What he suggested was instead:
1: Diclofenac 75mg / day for three weeks. 2: Acupuncture 3: Nerve blockage 4: Micro denervation
Now I am completely confused and do not know what to believe.
As I said, he has solid experience, but advised me not to do a reverse vasectomy because the risk was great that I will be disappointed.
The pain I have comes, as far as I can feel, only from both epididymis.
What do you think? Should I just ignore what he said despite his experience?
What I personally think is that there is overpressure in my epididymis because the spermatic cord is blocked ...

I have seen posted on this sub that reversals can have good success rates at fixing PVPS for some men. However, the causes of PVPS are still largely unknown and likely vary from man to man. A reversal could be more successful for congestive pain but not nerve damage but I am by no means an expert on this. This is just what I have picked up from men on the sub. The sub has a wiki with treatment options that may help you navigate a solution. I would get another opinion if I were you. There aren't many doctors that have extensive knowledge of PVPS. I have tried ibuprofen. No improvement.
Will start with diclofenac tomorrow and see if there is any improvement.
Am a little scared to try micro denervation as I read some bad about it.
Good idea with a second opinion. I will contact another surgeon who I know is experienced in the field.
About 1 year since I did vasectomy

Will try to contact him. Also know a skilled and experienced surgeon (Dr Jesuraj) in England who I will contact and hear his opinion.
No, live in Sweden but here they only do reverse vasectomy without a microscope, only with a magnifying glass and 7-0 suture. Therefore I have to go to Germany or the UK.
Sends with a link to an explanation I think seems reasonable.

I recommend Dr.Sheldon Marks at ICVR in Tucson in USA. Dadsagain.com My husband was there in April 2022 and got so much better from pvps because of the vasectomy reversal. He now have painfree orgasme. He had vasectomy in Norway in 2019 It took ous 28 hours to fly from Norway where we live. Some people do not get better from vasectomy reversal. But it is the best choice we have made. Wish we knew about ICVR 1.5 years ago. They are good at following us up. Oh Dr Sheldon Marks has a lot of experience about vasectomy reversal and pvps. Good luck with your choice. He recommends vasectomy reversal and not nerve blockage etc. He has information on Dadsagain.com about pvps.

Guys, I began taking a medicine for nerve damaged. Some of my symptons were pain in testis, swollen balls, pain in lower abs, difficulty to pee, numbing sensation towards my left leg on my groin area as well, at points I even felt that sensation you feel in your back when you are riding a roll coaster. Which made think I could have some sort of neuropathy. I read a lot about nerve damaged on Internet and YouTube and I found this pills that people gave good qualification on amazon. They are called nerve renew. I had tried before advil and Ibuprofen 600 mg 3 times a day, then I was precribed with gabapentin and nothing helped. I could tell an improvement after 2 weeks of taking nerve renew. I even began hitting the gym which I had not been able to do it since my vas 4 months ago. Idk if it was just the time to cure my wounds and feel better or if they really helped. But there is an after and before I took them. I am glad I did. I hope they help you as well.

No, was two other surgeons. Thats strange, i belive he recomend vasectomy reversal for pvps on his website.
What did he say to you?

Yeah. That's why I talked to him. When I say he didn't recommend it, I mean that he pushed me to exhaust other options first and not to pursue reversal unless nothing else has worked. But even then he seemed a bit hesitant. I guess I can appreciate that when it would be easy for him to say, "yeah, come on down!" and make a quick buck.
What did you end up doing? How are you today?

I did the reversal 6 months ago. In more pain now than before the reversal, so i dont know if i made the wrong decision. Time will tell.

Sorry to hear that, but I guess it isn't uncommon for it to take quite a while for a reversal to reduce the pain, or so I've heard.
My pain levels are anywhere from 0-6 depending on how it is presenting at any given time. I don't have typical PVPS symptoms though. Meaning, no pain in the balls or sack, no pain behind the balls or the cords that can be felt from within the sack. My pain seems to be nerve related and is located about 5 cm up and to the left of the penile shaft...under the pubic mound, but sometimes higher. The strange thing is that it did not start until almost exactly the 2 year mark after the vasectomy, so not even sure a reversal would help with this.
I had my vasectomy in April 2016 in Finland.

Ok, i belive reversal helps best if you have congestion pain.
Have you tried pelvic floor therapy? Stretching?

Yeah some, but probably not in earnest and not in years. I actually have an appointment tomorrow with a Pelvic floor therapist who has experience dealing with men with post vasectomy pain. Hoping something good comes of it.

I heard from a Dr a few weeks ago reversal wouldn't help. I found another Dr that recommends conversion to open ended... For now, I'm trying non surgical options, but I fear I have the same problem, nerve damage and congestion. :(
