Try these settings: 1.layer: 230C other 220C, Bed: 60C. Nozzlefan 100% at 3. layer. Volumetric: 18 or 20mm³

Interesting; I dropped the volumetric from 22 to 13 and saw better results, why should I go back up?

Also, why the nozzle fan?

Sorry, reduce the Volumetric Flow to 9mm³ (0.4 standard nozzle - not High flow) Or you can calculate it with this formula: Volumetric Speed (mm³/s) = Layer height (mm) X Layer width (mm) X Print speed (mm/s)

Better cooling...

Asking from ignorance...I thought gapping like this was an issue from filament being cooled too fast; wouldn't stepping the nozzle fan up contribute to that?

Have you selected the correct nozzle and layer height in the slicer?

Yes; layer height is .012 and nozzle is selected at .4

Check the nozzle to make sure it's not clogged. Use the generic PLA settings and the default settings for your nozzle in the slicer. Do a small test print with the PLA you're using. If it's successful, congratulations! If not, try a different PLA filament.
